How to Submit Your Samples
Review the process below to make sure your samples get to the correct lab manager.
- Ensure you have a valid agreement. Instructions can be found here.
- Once you have an agreement, submit your sample details here. You must have an agreement before you can complete this form.
How to login to submit this form.
If you’re an NC State user, login with your Unity ID. If you’re an external user, click the Guest Login button. If this is your first time logging into our Lab Management System, Mendix, as an external user, click Forgot Password and enter your email address. You’ll then receive an email to create a new password which will enable you to login.
- Provide your sample(s). For local users, samples can be dropped off in the locker outside of MRC 318. Make sure your name, email address, and Attn To [lab manager’s name] are listed on your sample(s). To drop off CT samples, please drop them off at this address: 911 Partners Way (Engineering Building 1), Raleigh, NC 27606, on the table in front of room 1046.

- Pick up your sample(s). For local users, samples can be picked up from the locker outside of MRC 318. Please pickup your samples within 1 month or they will be discarded. For non-local users, email the lab manager with your mailing information. Your sample(s) will be returned via FedEx.
